ID Card Signup Form & Payment

The Discount ID card entitles you to discounted green fees at both Hawthorne Hills and Mee-Kwon Park Golf Courses. See Rates & Fees to view the discounts. There is an annual fee each year which is based on age and residency. You may purchase the card by filling out the information below or at the time of check-in at either golf shop. The email you provide will enable you to sign up for Tee Times on our website up to 14 days in advance.

Proof of age and residency via a valid Driver's License and/or Student ID Card must be presented upon Check-In at the start of your season
Deceptive information to receive discounts not permitted and will result in restitution for monies lost to Ozaukee County
